General Statement of Duties

Provides professional case management services for children, adults and families in the foster care and/or adoption unit; exercises independent judgment and initiative and is thoroughly familiar with agency standards, policies and functions as well as the full range of community resources; performs other duties as assigned.

Essential Duties

A Family Service Specialist II in the Adult Service team partners with a nurse from the local Health Department to conduct long term care screenings in the individual’s home to address a wide array of service needs within our community. 


Essential Duties:


  • Complete long-term care screenings for children and adults to include:
    • Review initial screening requests 
    • Conducting home visits with health department staff to complete screenings and make eligibility determinations 
    • Follow-up with clients after screenings are completed to ensure services are in place
    • Complete all necessary documentation for correspondence, case approvals, and appeals per State and Local policies;


  • Assist customers with the Medicaid application process by helping them fill out applications, sending out verification checklists, and collecting verifications for the Medicaid determination.


  • Assess individuals seeking to enter or to remain in assisted living facilities using the Auxiliary Grant.


  • Participate in community, agency and division committees and task forces as needed for specific projects, or for ongoing functions;


  • Mandated participation in Henrico County Emergency Shelter Operations as designated by the Emergency Operations Center and the county manager; and


  • Perform related work as assigned and accept additional assignments when necessary. 


This position requires strong knowledge of Adult Service Screening policies, as well as general knowledge of Medicaid policy, Adult Protective Services policy, and available community services. The role also requires proficiency in social work principles and practices, an understanding of human behavior and current social, economic, and health issues, and strong oral and written communication skills. Candidates must be able to plan and organize work efficiently while demonstrating empathy and objectivity when working with clients.

Additional Information

Valid driver's license to perform assigned duties at various locations. ·

Successful completion of all training requirements established pursuant to the VA 63.2-208. Standards for personnel. ·

Central Registry Check and Criminal history record check and fingerprinting of all employees in authorized and hourly safety sensitive positions.

State or Federal Funding Clause
This agreement and the continuation of this position are contingent upon the availability of specific special revenue funds.

Minimum Requirements

Four (4) year baccalaureate degree in a relevant human services field in accordance with VA Code § 63.2-219, and two (2) years of relevant social work experience; OR, any equivalent combination of experience and training which provides the required knowledge, skills and abilities.
